If you do not bring these documents, the consultation or vaccination will be considered private (self-paid).
If you are more than 10 minutes late for your appointment, your reservation will be automatically cancelled.

Our staff mainly speak Japanese. Please bring someone who can help you communicate in Japanese or English, if necessary.
